Dispute forms

Use the forms below for account disputes such as transaction or merchant invoice disputes. Please note: for all scenarios in which you believe fraud has occurred, we recommend you always cancel your card and request a new one. You can do so through the online portal or by calling our customer service team.

EFS and Fleet One customers

Use this form to file a claim to contest part or all of a transaction.

Use this form to submit a claim to dispute a transaction you believe to be fraud (including lost/stolen cards).
